Sample Medical Justification – Static Footrests
The following are sample medical justification wording for the Seating Dynamics Static Footrests which may be used in documentation, such as a Letter of Medical Necessity. Justifications should also be customized for a specific client’s needs. These examples do not replace competent evaluation. While manual and power wheelchairs are available with a variety of footrests, standard footrests do not meet the need of everyone. Footrest placement is sometimes limited by client range of motion loss and orthopedic asymmetries, particularly when the hamstrings are very tight. If range of motion limitations or orthopedic asymmetries prevent the client’s foot from being in alignment with a standard footrest and footplate, our static footrests can accommodate a variety of positional needs.
The Static Adjustable Knee Option allows unlimited adjustment in 7.5 degree increments when standard footrest angles are inadequate to meet a client’s needs.
Seating Dynamics Static Footrests with Static Adjustable Knee Option are being recommended as standard available footrest angles are not adequate to match the required knee flexion angle due to range of motion limitations.
The Adjustable Super Contracture option is for clients with very limited knee extension.
Seating Dynamics Static Footrests with Adjustable Super Contracture option are being recommended to accommodate atypical foot placement due to limited knee extension. Standard footrests cannot support the feet in the required location.
The Adjustable Windswept option is for clients with a windswept tendency or excessive bilateral hip abduction.
Seating Dynamics Static Footrests with Adjustable Windswept option are being recommended to accommodate atypical foot placement due to (a windswept posture) or (bilateral hip adduction) or (bilateral hip abduction) (choose appropriate option). Standard footrests cannot support the feet in the required location.
The Adjustable Plantar / Dorsi flexion footrest extension is for clients that require more adjustment into plantar or dorsi flexion than is available standard on manual and power wheelchairs.
Seating Dynamics Static Footrests with Adjustable Plantar / Dorsi flexion extension are being recommended due to ankle range limitations to accommodate the feet in a greater degree of dorsi or plantar flexion than can be supported by standard footrests.
Manual and Power Wheelchairs include footrest hangers with footplates as a standard option. The footrest hangers typically swing-away and the footplates flip-up to move out of the way for transfers. There are times when standard footplates do not meet the needs of an individual client and a footboard is required instead. The Seating Dynamics Footboard can be used on numerous wheelchairs and flips-up to accommodate transfers. The One-Piece Footboard is available for any Seating Dynamics Static or Dynamic Footrest. The One-Piece Footboard only flips-up on the Static Footrest, at this time.
Depending on available range of motion or contractures of the knee and ankle joints, a client’s foot position may not align with a standard foot plate position. A footboard provides a wider surface to accommodate unique foot placements. Other Seating Dynamics hardware can also be used to accommodate these needs.
Sometimes standard footplates chronically sag down medially, leading to foot pronation. The One-Piece footboard can be used to prevent this.
