seating dynamics blog dear cms tamara's thoughtsIn 2022, CMS published a proposed fee schedule for Dynamic Backs and many people answered the call to provide comments on this important topic. Unfortunately, CMS decided not to receive comments right before a public meeting on this topic, stating that a final decision had already been made. We have been periodically sharing comments that were submitted so these voices can be heard.

Tamara is a good friend and an amazing colleague. A true pioneer in postural care, she now heads up the Clinician Task Force as Executive Director. When the call went out to provide comments to CMS, she submitted her valuable insight!

“I have specialized in seating and wheeled mobility for 38+ years. Over those decades I worked with many individuals who had complex needs. Some of them had very high muscle tone or spasticity, sometimes causing their entire bodies to move into strong extensor spasms. Others habitually rocked in their wheelchairs. Some had severe seizures from time to time, causing them to slam into their back and head supports.

Some of these individuals could exert enough force on their wheelchair frames and seating system hardware to break back canes. The people I worked with definitely loosened back support hardware out of adjustment and even broke the hardware. Some also rocked hard enough that the entire wheelchair tipped backward, putting them and others at risk for injury.

I was able to resolve all of these problems by using dynamic backs on these clients’ wheelchairs. This intervention saved their wheelchairs from damage and expensive, time-consuming repairs, and protected them from injury. Only a small percentage of wheelchair users need dynamic seating, but when needed, a dynamic back is essential for their health and quality of life!”.
