Dear CMS…a Clinician’s Perspective

In 2022, CMS published a proposed fee schedule for Dynamic Backs and many people answered the call to provide comments on this important topic. Unfortunately, CMS decided not to receive comments right before a public meeting on this topic, stating that a final decision had already been made. We have been periodically sharing comments that were submitted so these voices can be heard.
Karla Sonderland, MS, OTR/L, ATP is an Occupational Therapist with over 20 years of experience in seating and wheeled mobility. She now is a Clinical Education Manager for Sunrise Medical.
“I would like to provide clinical input as to the necessity of dynamic backs and why these need to be reimbursed at an appropriate amount.
I have worked with many individuals that have tonal influences and sensory impairments impacting their wheelchair positioning and functional abilities. Increased muscle tone and sensory seeking behaviors often create excessive force on the wheelchair frame and seating hardware, increasing the risk for breakage and subsequent unavailability of proper supports. Dynamic backs are designed to lessen the impact of client forces, therefore increasing the integrity and availability of necessary postural support for the individuals. Though this may not be a high demand item, when it is needed, it's needed for the safety of the individual and their caregivers.
I encourage further discussion on the importance of a safe and strong dynamic back that will withstand the significant and repetitive forces of children and adults for whom these backs are appropriate and needed for their health and safety.
